Kindergarten Registration Information

School Town of Highland- Kindergarten Registration Information

Physical and Immunization Requirements:

All students in Kindergarten are required to have a Physical examination by a licensed physician and a copy of their immunization records turned in prior to the first day of Kindergarten. Students will have 20 days after the first day of school to turn in required documents. If items are not turned in within 20 days, the student will be excluded from school per Indiana law.

Dental Requirements:

All students in Kindergarten are required to have a dental examination prior to their 1st day of school. The dentist should complete the documents listed below. Please turn in to your student's teacher or to the Nurse's office.

Students with Health Needs

Students with a diagnosis of Life Threatening Allergies, Asthma, Diabetes, or Seizures are required to have completed health care documents turned into the Nurse's office at the student's designated schools. Please see the documents below that list what is needed for each diagnosis. Any students with a completed action plan, must also have a Medication Authorization on file in the Nurse's office.

Should you have any questions, please contact the Nurse at your student's school.

Medications at School

Should your student need medications at school, please complete all of the documents in the packet below. A Medication Authorization form must be completed by the Family Physician if a student is going to take prescription or emergency medications at school. A Medication Authorization form must be completed by the parent for any over the counter medications that will be taken at school. All medications are to be brought in by a parent or guardian. All medications are stored in the Nurse's office at your student's school.
